About United Community Health Center
UCHC is a 501(c)(3) non-profit agency that relies on support from individuals as well as corporate gifts and grants, charitable donations and program contracts. What does it mean to be a federally qualified health center?
- A federally qualified health center means the federal government recognizes UCHC meets a high standard of clinical care and makes its services available to all people.
- Receives annual funding from the Bureau of Primary Health Care.
- Is an independent non-profit corporation governed by a consumer-dominated Board of Directors.

Outreach
UCHC’s Outreach is dedicated to building healthier, more informed communities through education, service, and support. Throughout the year, our team actively engages with schools and community organizations to provide educational materials and interactive demonstrations that help children and youth learn about health, wellness, and prevention in engaging and age-appropriate ways.
Beyond education, UCHC volunteers its time to support local initiatives that address immediate community needs. Our outreach efforts include food donations and the distribution of essential care items such as condoms, mental health resources, and dental hygiene supplies. These efforts are designed to reduce barriers to care, promote preventive health, and support overall well-being.
Through collaboration, service, and compassion, UCHC Outreach works to create lasting positive impact and strengthen the communities we serve.

- 28 rides provided to individuals in need of transportation to healthcare appointments.
- 233 telehealth visits were conducted to ensure continuous access to care, regardless of distance or circumstance.
- Services delivered to 79 distinct ZIP codes, expanding our reach across diverse communities.
- 25+ veterans served, honoring our commitment to those who have served our country.
